Please Fill out these forms before July 15th!
1. Medical Consent Forms – Without 2 copies of the notarized form and insurance card (front and back), the band member cannot participate in band activities. (A notary will be available at the July 30, 2009 Booster Meeting if you are unable to get the forms notarized before then)

2. Band Member Information Form – This provides us all with valuable internal/external communications information we have to have.

3. Fees Financing Contract – YOU WILL NOTICE THAT BAND FEES FOR THIS YEAR ARE THE SAME AS LAST YEAR AND THAT WE EXTENDED PAYMENT PLANS OUT TO FIVE MONTHS. Please – indicate which of the three financing option you wish to exercise, sign, date and return it to us as soon as possible. We have to have those contracts to plan what we need to do to meet budget and to set up some of the fundraising options available to us. If, as the contract indicates, you believe you will be unable to meet the payment/work off requirements for your child you need to contact Mr. Cox immediately to see what alternatives are available to you.
